The lips are supplied by the superior and inferior labial arteries. Both branch from the facial artery, and both branch again once they reach the lip itself, creating a dense network of vessels running through exactly the tissue being treated. A needle passing through that tissue therefore has a realistic chance of catching a small vessel, and a bruise is the visible result.
That same blood supply explains the swelling. Fluid moves readily into tissue this well perfused, so the volume visible on the second day is partly product and partly the body’s response to being injected. This is a structural characteristic of the area rather than a complication, which is why the goal of preparation is to limit the reaction rather than to prevent it.
Alcohol and hot drinks both dilate blood vessels. In an area already carrying this much circulation, that widening increases the likelihood of bruising and can make the early swelling more pronounced. Leaving both out in the run-up to an appointment is a small adjustment with a visible return.
Blood thinning medication is a separate matter and is handled differently. Patients taking it are asked to check with the doctor who prescribed it rather than pausing it themselves, because that medication is prescribed for reasons that outweigh a cosmetic bruise. The question is raised at consultation and confirmed before a treatment date is set.
Cold compresses are the first measure. Cold constricts the small vessels near the surface, limiting both bruising and early swelling, and they are applied gently and intermittently rather than pressed hard or held continuously. The aim is to calm the tissue, not to compress the product that has just been placed.
Intense exercise is avoided for the same underlying reason. Raising the heart rate and blood pressure pushes more blood into the face, which makes swelling and bruising more obvious in the days immediately after treatment. Staying well hydrated supports recovery alongside this, since the hyaluronic acid used in most lip fillers draws and holds water.
Day one lips are swollen lips. The volume is exaggerated, the shape can look uneven while swelling settles asymmetrically, and the border can appear firmer than it will be. Assessing the outcome at that point, or photographing it and comparing it against someone else’s healed result, gives very little useful information about how the treatment has actually gone.

The shape a patient keeps is therefore the shape visible at the end of that period rather than at the beginning, which is why review is scheduled rather than left to the mirror. Patients whose concern is volume, definition or proportion in the lip tend to be well suited to lip filler, provided the surrounding structure supports the change being asked for. Assessment covers the lip in the context of the rest of the lower face rather than in isolation, since a lip that looks out of balance is not always a lip that needs more product.
